Welcome to Junior High! In order to participate in athletics both here and at the high school level, students need an Iowa Athletic Pre-Participation Physical Examination Physical Form completed each year. The physical is good for 12 months with a 30 day grace period. I encourage you to schedule your appointment early in the summer to avoid the August rush. PHYSICAL FORM

Varsity Bound 

Incoming freshmen are required to have a Baseline Concussion Test completed if they are participating in any contact sport. Contact sports include: football, volleyball, basketball, wrestling, soccer, baseball, softball, cheer and dance athletes. Excludes are: cross country, golf, bowling, tennis, track and trapshooting. CONCUSSION TESTING FORM

Incoming freshmen are required to have a Certificate of Dental Screening/Certificate of Dental Screening Exemption on file with the High School Nurse, Jenna Dexter.

All students entering 7th grade must have proof of having both the Meningococcal and the Tdap vaccines prior to the first day of school in August (2022), unless the student has a Certificate of Immunization Exemption on file. Read more in the letter below.
