Welcome to Junior High! In order to participate in athletics both here and at the high school level, students need an Iowa Athletic Pre-Participation Physical Examination Physical Form completed each year. The physical is good for 12 months with a 30 day grace period. I encourage you to schedule your appointment early in the summer to avoid the August rush. Download the form below and take it to your doctor's appointment with you.
