Welcome to Junior High! In order to participate in athletics both here and at the high school level, students need an Iowa Athletic Pre-Participation Physical Examination Physical Form completed each year. The physical is good for 12 months with a 30 day grace period. I encourage you to schedule your appointment early in the summer to avoid the August rush. 7th Grade Immunization requirements 

The Iowa Department of Public Health requires all students entering 7th grade to have both the Tdap booster and Meningococcal vaccination prior to starting 7th grade. Please drop off any updated immunization records to the main office at Orientation Night or Open House.
